Details:
The Program Director will provide administration and supervision to the Health Home Care management program. The Program Director will provide direct supervision to program staff to ensure the highest quality care management services are delivered in the most efficient and effective manner. The Program Director will represent the program at external meetings with the Lead Health Home, and governmental agencies and community resources to ensure Acacia Network Health Home adopts operational processes to ensure optimal programmatic and financial functioning. Additionally, the Director will ensure that administrative and financial processes and policies and procedures are implemented and adhered to by program staff to ensure compliance with Acacia Network, Health Home and regulatory policy. Salary range is $80-90K
KEY ESSENTIAL FUNCTIONS:
- Create program policy and procedures and conduct periodic reviews and updates as needed to ensure program is in compliance with Acacia Network, BAHN, and governmental regulations and best practices.
- Directly Supervise Clinical Supervisors, Operations Coordinator and indirectly supervise Health Navigators, Data Team, and Care Managers to ensure the delivery of high quality, efficient care manager services and billing.
- Lead and participate in performance improvement or quality improvement activities, projects and reviews.
- Interacts with external and internal stakeholders and represents the program, departmental and agency positions, concerns and responds professional with the goal of shared, mutual understanding.
- Identify new funding sources and in conjunction with departmental agency and staff, develop responses for continued programmatic support, program expansions and new programs.
- Meet regularly with staff in team meetings and quarterly staff meetings and discuss process and clinical expectations, clarify expectations and document these discussions/meetings with agenda, meeting minutes, etc.
- Complete monthly, annual, or other periodic request for narrative or quantitative data reports for funding sources or internal departments.
- Ensure services are delivered in accordance with contractual agreements with funding sources and/or agency protocols.
- Follow all agency policy and procedures in staff management including hiring, corrective action, talent evaluations, professional development plans, etc.
- Meet with consumers individually or as a group to discuss consumer satisfaction and opportunities for improvement as well as program strengths.
- Assess and respond per agency guidelines to client complaints and grievances.
- Lead departmental meetings to discuss and assess current operations. Gather input from staff and implement process changes to improve service delivery.
- Follow agency corrective action policy when needed to ensure staff performance meets the n=high expectations of the agency
- Complete probationary, interim and annual reviews.
- Communicate effectively with Administrator and Executive Management in identifying strengths, weaknesses, and opportunities of program operations.
- Identify staff training needs and gaps and locate training resources. Ensure staff attend mandatory training. Implement and maintain system for tracking staff attendance at training.
- Maintain coherent and comprehensive tracking systems to ensure service deliverables and financial goals are achieved including the accuracy and timelines of entry of contacts into the BAHN portal
- Oversee the development of promotional and program written material and periodically evaluate for effectiveness; make recommendations for improvements.
- Conduct and oversee internal and external outreach efforts, including daily tracking logs and other tracking and administrative tools and reports; create and implement new systems as needs are identified. Review entry contacts entered into the BAHN portal and ensure accuracy of billing component to prevent bill submission errors.
- Oversee and implement efficient referral and assignment process from initial assignment through engagement process.
- Organize staff responsibilities to ensure maximum contacts as defined by Health home and regulatory bodies each billing cycle.
- Attend BAHN meetings to discuss program operations and implement new processes and/or policies and procedures as indicated
- Review program financial targets monthly and Administrator with periodic updates on variances and suggest changes to maximize revenue
- Implement utilization of RHIO and other electronic health resources and develop processes to integrate utilization into care management policy and procedures.
- Integrate Criminal Justice population with Health Home services in accordance with the BAHN. Attend informational case conferences and provide bottom up referrals when appropriate. Provide BAHN with minutes of case conferences.
REQUIREMENTS:
- Two (2) or more years of progressively responsible experience in operating or supervising a care management program
- AA/AS NYS Licensed Practical Nurse
OR
- Licensed Masters Social work or eligible to be licensed within six (6) months
- Desire to provide quality mental health services and develop the administrative and clinical staff
- An understanding of funding and industry regulations and a track record of meeting compliance standards
- Strong leadership, excellent organizational and communication skills
- Ability to work on a strong team of professionals in culturally diverse environment
- Ability to multitask and to work and function under pressure
