General Description

Helps individuals with intellectual and developmental specials needs in establishing their Individual Directed Strategy (PDP), which determines services & supports required to acquire life objectives. Displays customers’ development towards objectives & goals and complete satisfaction with services & supports readily available through the House and Community-based Solutions program. This is finished through regular monthly face to deal with contact & regular telephone contact with the people appointed, their households, and their provider. The work needs extensive coordination of personal companies’ services and offered neighborhood resources consisting of Medicaid, Social Security, and other neighborhood based programs where readily available. Needs interaction and cooperation with numerous internal departments within Nueces Center for Mental Health and Intellectual Disabilities. Minimum Certification

A certified expert with proper medical training and experience with a minimum of a bachelor’s or postgraduate degree from a certified college or university with a significant in social, behavioral, or human service field consisting of however not restricted to psychology, social work, medication, nursing, rehab, therapy, sociology, human advancement, gerontology, academic psychology, education, and criminal justice.
